J.D. gets back with Danni after Elliot goes back to Sean when he returns.
Cox refuses to let a patient leave to play with his band who are about to embark on a European tour. He bickers with Dr. Miller, before realizing that he has a crush on her. He arranges for his band to come into the ward so that he can perform with them. During The Polyphonic Spree's performance of "Light and Day", Grace invites Cox for a drink after work. He goes with Jordan instead, who has dyed her hair blond.
Sean asks J.D. if anything had happened between him and Elliot while he was away. Considering his friend, J.D. says there had not.
This is the only appearance of Irv, the kindly old overweight security guard who is also a friend of J.D. J.D. watches him kindly offer to park a man's car for him, only to be insulted because of his weight problem. He is later "let go" after trying to suffocate the man with a pillow.
Turk and Carla encourage the Janitor to apply for Irv's position of security guard. Janitor insists on finding out who disconnects the alarm for the fire exit. It turns out to be Kelso. He is worried that Kelso will fire him if he stops him, but he later charges at Kelso and knocks him down. In time with J.D.'s narration about being proud of the decision you make, Janitor, now back as a Janitor, is seen happily mopping the floors.

- In the scene where Irv is tying to smother a patient, Carla "injects" a sedative into Irv's shoulder and he falls to the ground. However, she never actually depresses the plunger. In essence, she simply stabbed him with a needle.
