The episode begins with JD walking into the hospital, passing The Todd who says he is waiting “his moment”. JD continues walking and asks Dr. Cox if he can go to lunch with him and is immediately shot down. JD denies the fact that Dr. Cox does not like him. The Todd reveals that he lies in wait for people to make comments that could be seen as a sexual comment, and that it's hard work. JD and Dr. Cox meet at a grocery store (JD followed Dr. Cox) and are confronted by former patient Jill Tracy. She nags on about how she was stood up for a date. Dr. Cox abandons JD to the annoying Jill.

The second act begins with JD walking into a room full of patients, revealing they all need transplants . One of the patients needs a kidney transplant. He is David Radford, a friend of Dr. Cox. Dr. Cox talks to the hospital staff about helping people. This is what motivates him, and Dr. Cox tells the doctors how to deal with each patient. It then cuts to JD who is picking up food for everyone and notices Jill Tracy again. He tries to avoid her but she catches him and he is stuck in a conversation. At the hospital, the Todd pops out to comment on a sexual innuendo during a conversation between Elliot and Carla. The girls then discuss the women the Todd has had sex with - it turns out he never had sex with any of the women he claims to. Carla tells JD and Turk how the Todd may be gay. Dr. Cox is given the bad news that the patients in need of transplants are failing; he tries to motivate the staff. JD walks by and is told Jill has been re-admitted and is positive for cocaine. It then cuts to a montage of Turk seeing the Todd being gay and Dr. Cox getting new organs after Jill dies.

After successful transplants, Dr. Cox - in a rare happy mood - thanks the staff. JD is still depressed about Jill's death, but Dr. Cox finally invites him to lunch. Turk, Carla, and Elliot continue discussing the fact that the Todd is gay. JD and Dr. Cox eat their lunch and discuss Jill. Dr. Cox mentions how outside the hospital it's not their job to help people. Dr. Cox gives words of wisdom to JD: “once you start blaming yourself for deaths that are not your fault, it’s a slippery slope that you can't come back from.” Carla and Elliot talk to the Todd about his homosexuality and are pleased that he has come to grips with himself as a new person. Unfortunately, the Todd is as big a jerk as before; now for men. JD and Dr. Cox come back from lunch where they learn everyone who received new organs is failing. Things turn for the worse, and it is revealed that Jill had rabies - in turn, all the donor parts are infected. Elliot and Carla talk to the Todd about being a pig. It turns out the Todd was scamming the girls with the idea that "chicks dig gay guys." The Todd walks down the hallway and reveals he is attracted to everyone and everything. The Janitor asks the Todd "What are you?", to which the Tod simply replies, “I’m the Todd”. A montage begins, involving all the donor patients dying from their new rabies infected organs. JD tries to cheer up Dr. Cox with lunch and tells him it wasn’t his fault, seemingly making him a little happier. Before they can eat, their pagers go off. The emergency turns out to be Dr. Cox's friend, who he desperately tries to save. When the patient dies Dr. Cox suffers a breakdown, throws down the defribillators and screams in anguish. Dr. Cox reveals to JD that his friend could have waited a month for the kidney, but that he was focussed on getting all three patients organs as soon as possible (the other two could not wait). Because of this he blames the deaths on himself and walks away. JD reminds Dr. Cox that "the second you start blaming yourself for people's deaths, there's no coming back", but Dr. Cox merely says "you're right" and leaves the hospital.

This episode is the second-highest-rated episode of Scrubs in TV.com (9.72 out of 10) preceded only by "My Screw Up" (9.80 out of 10) and followed by "My Way Home" (9.65 out of 10.). It is also the second highest-rated episode in IMDB (9.1 out of 10) preceded only again by "My Screw Up" (9.5 out of 10) and followed by "My Best Friend's Baby's Baby and My Baby's Baby" (9.0 out of 10).
In the sixth-season episode "My Long Goodbye," Dr. Cox sees the ghost of a pregnant Jill Tracy, who died in this episode.
