J.D. is having problems dealing with 'train wreck' codes - when a patient is flat-lining and lots of other problems occur at once. Elliot is very good at handling these situations. Initially J.D. doesn't worry about Elliot's superiority because he believes he has the upper hand at diagnosis. However, when Elliot diagnoses a patient before him, J.D. is forced to wonder if she is a better doctor than him.

- The name of the episode is a play on the medical term 'Myocardial Infarction' (heart attack).
- The sidestory with the Janitor trying to convince Elliot to drink coffee with him is more or less a continuity error. At the end of the episode before, My Best Moment, we see Elliot casually drinking coffee with the Janitor while telling him her best moment in medicine.
- This episode features the second onscreen appearance of the oft mentioned Nurse Tisdale (the first appearance being in Season One, Episode 11: "My Own Personal Jesus").
- There is also another appearance of "the world's most giant doctor", with Elliot on the bottom and J.D. on top, and at the very end of the episode, they make "the world's most most giant doctor" after Elliot shows she can break a lunch tray between her thighs. They are joined by Lonnie, an intern, to accomplish this.
