Resident Evil: Survivor (Japanese:Biohazard: Gun Survivor) is set in the isolated city of Sheena Island around November 1998. This game is a major departure from the series, deviating from the third-person perspective of the previous games to a first-person view. The Japanese and PAL versions of the game allowed use of the Namco GunCon, a light gun that gave the game a more arcade feel. The American version did not allow the use of the GunCon. Players have often found that the use of a GunCon increases the enjoyment and playabilitty of the game dramatically.

In September, 1998, a disaster struck the mid-western American town of Raccoon City. An outbreak of the T-virus transformed the city's populace into zombies. As a final desperate measure to halt the spread of infection, the U.S. government approved an air-to-surface missile strike on Raccoon City, annihilating the doomed town and its entire population in the process. Not long after this incident, a helicopter crashes on the outskirts of the Umbrella Corporation's private township, located on Sheena Island. The pilot, Ark Thompson, escapes the burning wreckage only to find himself fighting a battle against the living dead, with no memory of his identity or his reasons for being there.

The player immediately takes control of the amnesiac protagonist, struggling desperately against overwhelming odds to stay alive and uncover his identity. Survivor's gameplay can be described as a combination of first-person shooter and arcade-style gun games such as the House of the Dead or Time Crisis series. As is generally the case in all first-person shooter games, the player views the environment from the perspective of the character and moves said character through the environment with the controller. At the same time, if the player wishes to attack an enemy or destroy an object, they depress a button which allows them to control a crosshair on the screen. This crosshair can be moved to any location within the main character's line of sight.

The game borrows its enemy rosters from Resident Evil 2, featuring the same zombie models from that game, as well as creatures such as Lickers, the Ivy plants, Moth Giants and even a few mass-produced Tyrants. In addition, Survivor also reintroduces the original Hunters from the first Resident Evil and features two new enemies in the form of the U.T. Unit (Umbrella Trashsweeper or UnderTaker), artificially-created soldiers armed with machine guns and the Hypnos T-Type, a new Tyrant model that evolves into different forms.

The player's arsenal consists almost entirely of pistols, such as the Glock 17 and the CZ75. As the protagonist progresses through the game, the player receives larger weapons to aid in defending against ever more dangerous opposition while he makes his way through Umbrella's ruined city and shadowy research labs.

The game features branching paths that allow the player to determine how the story will unfold and who they will meet along the way. Most of the paths, however, do not directly affect the storyline or outcome of the game.

Resident Evil: Survivor is often cited as being far less challenging than any of its predecessors in the Resident Evil series; a seasoned "survivalist" can, on average, finish the game in under 50 minutes. This, combined with the distinctive difference in gameplay as opposed to its 'survival-horror' relatives, has lead to the game being ridiculed or simply ignored by the most diehard fans of the Resident Evil franchise.^{[citation needed]}

Beta Hetero Non Serotonin is a fictional chemical mentioned through the events narrated in Resident Evil: Survivor, and is mentioned to be a special hormone secreted by the brains of teenagers during puberty, or little time before or after, with the property of being a prime necessity for the creation of the awesome biological weapons denominated Tyrants by the Umbrella Corporation, particularly the first-mass produced series, most notably the T-103.

The engineer who discovered these properties was Vincent Goldman, a ruthless Umbrella employee who used rather questionable methods to insure the free access of the corporation to the hormone, including global kidnapping and detainment of the kidnapped in the Umbrella-owned Sheena Island. He discovered various methods to extract the hormone, the most efficient of which was to instil deep-seated fear and alarm through the brains of the kidnapped teenagers, by extracting the master gland from within their brains without using any form of anaesthesia and refining the extract of the gland itself to extract the chemical.

Various ethnic groups were held in the facility's prison, in order to know which group could produce the largest amount of the hormone.
