Consulté con experto y me dijo que legalmente Carlos se llmaá realmente así. Por favor lean este texto que me respondió (en inglés):

Two of the sons of the Duquesa de Alba (Carlos and Jacobo) have legally changed their names from Martínez de Irujo y Fitz-James Stuart to Fitz-James Stuart y Martínez de Irujo. Their children have the surname Fitz-James Stuart. The same kind of legal name change has been done by Don Javier Fitz-James Stuart y de Soto, son of the Condesa de Requena (elder sister of the Duke of Berwick). Cuando escribí este artículo fue para adelantar trabajo para cuando Carlos Martínez de Irujo sea Duque de Alba. No estoy diciendo que Doña Cayetana vaya a morir ya, no. Sólo digo que este artículo es importante porque esta persona será el próximo duque. Por favor díganme que debo hacer para que no sea candidato a ser borrado. Ahora, si la justificacion de que será borrado es que el personaje tiene poca relevancia, lamento decirles que están equivocados...
