A Twist of the Knife was a Diagnosis: Murder TV movie, aired on February 13, 1993, on CBS. It was the third TV movie made, completing the trilogy of TV movies aired before the start of the series. A Twist of the Knife was preceded by Diagnosis of Murder and The House on Sycamore Street. It stars Dick Van Dyke as Dr. Mark Sloan, Barry Van Dyke as Steve Sloan and Cynthia Gibb as Dr. Amanda Bentley.

Dr. Sloan suspects Dr. Rachel Walters,the famous heart surgeon and one's of Dr. Sloan's old flames, has murdered US Senator Cabot during a guest operation at Community General Hospital. After a long investigation, Sloan and fellow detectives Dr. Amanda Bentley and Dr. Parker discover Rachel's motive. Years ago, Cabot had caused a hit-and-run accident that put her daughter into coma.
