As far as I know Louis Restivo (Massino's partner in Casablanca) was only a soldier. Massino was named official Underboss in 1981 after Sal Catalano stepped down, by 1982 he was the top power on the streets and the defacto Boss. Anthony Spero was the Acting Boss sanctioned by the Commission from 1987-93 while Rastelli and Massino were in jail, in 1991 Vitale became Underboss to Massino and was a co-leader with Spero, but from 1987-91 Spero was the Boss on the streets. The Acting Bosses, Committees or Panels started with the whole Bananas War bullshit in 1965 after Joe Bonanno stepped down as Boss in December of 1964. There was a 9-man ruling committee/panel for 3 months, then the Commssion apointed 3 senior capos (DiGregorio, Alfano & Caruso) to be the overseers of the family until DiGregorio was named Boss possibly in march, before June of 1965. Paul Scaicca was the acting or Street Boss from 1966 (sometime after Trautman St.) until DiGregorio retired in 1968. Anthony DiStefano claims that in February of 1969 there was a meeting in a Manhatten restaurant and Sciacca named underboss Frank Mari as Acting Boss and his successor, but that's impossible when Mari and Consigliere Mike Adamo disappeared along with a soldier in September of 1968. That's when Sciacca, the boss and leader of the old DiGregorio faction tried to bring the family together by making Bonnano faction leader, Natale "Joe Diamonds" Evola and Young Turk leader, Philip "Rusty" Rastelli Underboss & Consigliere. Evola was Boss from 1971-73 (after Sciacca and a capo were jailed on narcotics charges which shows Sciacca was active), Rastelli was named Acting Boss by the Commission from 1973 until he was officially named Boss in January of 1974. Carmine "Lilo" Galante is paroled in 1974, he waits until Rastelli is convicted in 1975 and becomes defacto Boss of the family since Underboss, Nicholas "Nicky Glasses" Marangello, Consigliere, Stefano "Stevie Beefs" Cannone and all the capos report to Galante and pay him tribute. Galante is hit July 12, 1979 and Salvatore "Sally Fruits" Ferrugia is named acting Boss by the Commission from 1979-83 until Rastelli is paroled. in 1987 Rastelli and Joey Massino are convicted and Tony Spero becomes Acting Boss from 1987-93 (Massino is paroled in November of 1992).
Within 2-3 years of Joey Massino becoming official Boss, Underboss Sal Vitale falls out with Massino by 1995 and is put on the shelf, then there are a number of Ruling Panels. From 1995-99, Capos, Anthony "Tony Green" Urso, Joseph "Joe Saunders" Cammarano, Gerlando "George from Canada" Sciascia led the Committee/Panel, Frank Coppa and James "Big Louie" Tartaglione help run the family as well. Sciascia is wacked in 1999, Capo, Richard "Shellackhead" Cantarella takes his place. By 2001 Cantarella is Acting Underboss, Urso Acting Consigliere, Coppa and Cantarella are jailed 2002 and Tartaglione is in Florida. After Joey Massino is arrested in January of 2003 the family is led by Acting Boss, Urso, Acting Underboss, Cammarano, Acting Consigliere, Anthony "Mr. Fish/Fat Tony" Rabito and Tartaglione comes back to New York to allegedly help, but he is an FBI informer and wired. in January 2004 Urso and Cammarano are jailed, Vinny "Gorgeous" Basciano becomes Acting Boss (this begins the Bronx faction's take over of the family) with Nicholas "Nicky Mouth" Santora as Acting Underbos and Rabito still as Acting Consigliere until Basciano is jailed in November of 2004, then Mikey "Nose" Mancuso takes over as Acting Boss, he is jailed in February of 2006 and now we have Salvatore "The Iron Worker/Sal the Zip" Montagna as Acting Boss while Santora and Rabito are currently under indictment!
