These cards can vary between company to company. Some have eligibility requirements, only specific pharmacies in their networks, and cover only certain drugs. These cards are not intended to be insurance, but are intended to help those that are insured get a break from retail drug prices.

1. Applicants must not have any prescription insurance or benefits through public or private insurance. If the applicant is eligible for Medicare Part D but chooses not to enroll then s/he can still enroll in this program. The applicant must also live in the US and be under the care of a US licensed doctor.)

3. My household income is equal to or less than: - $30,000 for a single person - $40,000 for a family of two - $50,000 for a family of three - $60,000 for a family of four - $70,000 for a family of five
