The film depicts Rambo (Sylvester Stallone) on a mission to Afghanistan. Colonel Trautman (Richard Crenna) has been captured while trying to supply weapons to the anti-Soviet Afghan Mujahideen, and because of the particular atrocities of the occupying Soviet army, only Rambo can save him. With the help of the Mujahideen, Rambo is able to rescue Trautman and eliminate the homicidal Soviet commander of that region. Ironically, Soviet troops were withdrawn from Afghanistan ten days before the film was released (starting in May 15, 1988).
A novelization was written by David Morrell, author of the novel First Blood, on which the first Rambo film was based.

- According to the Guinness Book of Records (1990) it is the most violent movie with 221 acts of violence and over 108 deaths. The film's gross in the United States was $10 million lower than the film's overall budget, but received a large gross worldwide and incredible income from video rentals.
- The Mi-24 Hind-D helicopters seen in the film are in fact modified Aérospatiale SA 330 Puma transport helicopters with fabricated bolt-on wings similar to the real Hind-Ds which were mainly used in the former Soviet bloc nations. The other helicopter depicted is a slightly reshaped Aerospatiale Gazelle.
- John Rambo's horse also appeared in Indiana Jones and the Last Crusade (1989) as Indiana Jones' horse.
- The Resistance Leader Massoud is the same last name as Ahmad Shah Massoud.
- At the end of the movie the words "This film is dedicated to the gallant people of Afghanistan" can be seen.
