[edit] Structure and effectiveness
The GyneFix 330 IUD consists of 6 copper sleeves (standard version) or 4 copper sleeves (small version or GyneFix 200), each 5 mm long and 2.2 mm in diameter, threaded on a length of polypropylene suture material. The proximal end of the thread is provided with a knot which at insertion is placed in the fundal myometrium of the uterus [womb] with an inserter for anchoring the device. This implant device has no plastic body, making it a completely flexible unit. Data on efficacy from large-scale, long-term international multicenter randomised and non-randomized comparative studies with GyneFix 330 (standard version) and with GyneFix 200 (small version) in parous and nulliparous women covering 15,000 woman-years of experience show that the standard GyneFix 330 is a highly effective intrauterine contraceptive system. Failures range from 0.0/100 users to 2.5/100 users (cumulative rates) during the first year up to 9 years of use. The longterm efficacy has been confirmed in a randomized comparative clinical trial conducted by the World Health Organization.
[edit] Acceptability of the frameless GyneFix IUD
GyneFix was designed to be superior to older IUDs by being less irritating (it has a smaller frame and contains less copper than other available IUDs) and being less-easily expelled from the womb due to its unique insertion method. The theory was that it would not increase the length or heaviness of menstrual periods as older IUDs did; would be more suitable for women whose uterine sounds have revealed a small uterus, and by extension more widely accepted as an appropriate form of contraception for nulliparous women (women who have never been pregnant). The development of the frameless GyneFix IUD is a response to the need to develop high-performing, long-acting, reversible and acceptable contraceptives with a high continuation of use and a minimum of menstrual bleeding disturbances. Based on long-term clinical trials the following conclusions can be made: The low failure rate of the securely anchored GyneFix IUD is attributed to the optimal target delivery of the copper ions in the upper part of the uterine cavity. Its performance is further optimized by the atraumatic design, which reduces partial and total expulsion and minimizes the side effects and discomforts experienced with conventional framed IUDs. The small GyneFix 200 IUD minimizes the impact on menstrual blood loss due to reduced size of the surface area of the foreign body while maintaining a high efficacy. Young nulliparous/nulligravid women may significantly benefit from the advantages of the frameless IUD, which could be strategically important to help reduce the increasing number of unintended pregnancies and induced abortions. Furthermore, the frameless IUD has been shown to be highly effective for emergency contraception and for immediate post-abortal insertion.[1]

Any provider committed to family planning is able to acquire proficiency with the anchoring technique in a minimum of time if the insertion instructions are followed strictly, a prerequisite which should be stressed.
