[edit] Episode overview
[edit] Act1
The episode beings with JD reminiscing about his lack of an apartment and how Turk and Carla are letting him stay there while he looks for one. Carla then tells JD he needs to find a place and they cannot let him stay here for much longer. Elliot talks about how she doesn't have a place to stay either starts to think to her about what she will do now that she no longer has a job as a doctor. She doesn't want anyone to know this, besides Carla who already knows. Turk suggests the two of them move in and the two of them reject the idea. The scene then moves to the hospital where JD reveals his birthday is coming up, or as he calls it “the anniversary of my 29th birthday” and his Mexican birthday party. JD then starts to narrate about how the surgeons are fighting for an attending position and Turk rushes off the scene to get the head surgeons brief case in order to get extra points to the Dr. Turk rushes outside to give him the brief case where a dream sequence beings with him and the Todd fighting surgical ninjas. After there victory of the ninjas Todd then betrays Turk, with a self-titled Betrayal Five, which actually happens much to JD's (the one who imagined the dream) joy.
[edit] Act2
Act 2 begins outside of the hospital where JD shows Turk the prank he performed, changing the name of the hospital to Sacred Fart hospital. They enter the building and Turk notices JD has accomplished none of his goals for things to do before he turns 30. It then switches to Turk alone talking to a patent who wishes to use hypnosis instead of anesthesia. Turk thinks this is ridiculous but Carla thinks that it could work but Cox thinks otherwise. Turk chooses to side with Cox who insults him and tells him “to always go with the wife, no matter how crazy they are” Carla is upset and tells Turk they have to work as a team and not argue. It then cuts to Carla and Elliot in the bar talking about what happened in the previous scene. The two notice a free table and manage to grab it… just as Dr. Cox and Jordan do. Carla tells Elliot that she should talk to her boyfriend Jake, because she is unsure if she wants him to move in with her. It then cuts back to Turk and JD talking about how JD has still done nothing on his list, while Turk has accomplished most of his. Turk starts to talk with Dr. Kelso who suggests Turk do something big in order to ensure he gets that internship. It then cuts to Elliot talking to Jake about opening up who then gives a bunch of strange and useless information. Elliot then suggests telling her his fantasy and she tells hers, which involves having sex with a Mexican apple thief. It then cuts to JD yelling at Doug for pre tagging patents, JD reveals to Carla his list and how he has accomplished nothing leading to his bad mood. Dr. Cox reveals to JD that there is something on his list he can do in time, a triathlon which JD decides to compete in. It then cuts to JD at the start of the triathalon and he cuts a montage, Turk agreeing to do the hypnosis surgery, but its not for Carla but because it will get him noticed. And Elliot seeing what Jake really is like and is scared out of her wits, it then returns to JD who is now running into the water but is to afraid to get in because its so cold.

Turk starts to speak to Carla and how he never believed in hypnosis and only did it to impress the head surgeon, angering her. It then re cuts back to JD doing the bike portion of the triathlon and Elliot pops up asking to talk to him about her issues with Jake. He tries to help and she rides off, JD then sees a sign that reads finish and rides of the hill, it then reveals the janitor painted the sign as revenge for JD messing up the sign that read sacred heart. It then cuts to JD on the last leg of the course and is shown being very far behind, some of the runners had time to cook a pig, in how over time he is. Elliot then re appears and tells JD how he freaked her out, and JD says Elliot is scared and that she has never accomplished something with her life and how Jake Is not right for her. A montage starts to run with Turk starting his operation, he makes a cut and the patent does not stir filling him with glee. It then shows JD walking it now dark and he is still not finished, and Elliot telling Jake her feelings and how she needs to talk. It cuts back to Turk and the patient awakens, while JD passes out on the ground. Elliot comes to JD’s aid and finds him stirring on the ground because of dehydration, and Elliot invites him to stay at her house.
