I've added some of the extensions I've done for our own wiki here for general use:

• MW:Extension:XmlWiki - A MediaWiki hack giving XML properties to articles
• MW:Extension:Simple Security - A simple to implement security extension
• MW:Extension:Tree view - Allows dynamic tree-views to be made with bullet-list syntax
• MW:Extension:Expandable tables - Allows expandable/collapsable tables to be created similar to WikiNews
• MW:Extension:Variables - Shows how to add new built in variables
• MW:Extension:DatabaseFetchObject - Dynamically adds a new hook to intercept database row requests

Here's some other extensions which work for our own environment but haven't been made to work in a standard MediaWiki environment yet

• OrganicDesign:Talk:pdf-export.php - can render categories as books
• OrganicDesign:Talk:email-article.php - send fully rendered articles including embedded CSS to email
• OrganicDesign:Category:ACTIONSCRIPT - integrating SWF with MediaWiki
• OrganicDesign:Category:Sandbox - integrating other scripting languages into MediaWiki
• OrganicDesign:Wikitext skin - Using normal wikitext articles to define skin content and layout
