Splatterhouse 3 is the title of a video game released by Namco for the Mega Drive/Genesis in 1993. It is the sequel to Splatterhouse 2 and was one of the first games to be given a rating by Sega's own Videogame Rating Council. The game was given a MA-13 rating by the council for its graphic violence and gore.

The game takes place about 5 years after the events of Splatterhouse 2, as Rick and Jennifer have since married and have a son named David. Rick has become a successful Parapsychologist in the tradition of his idol, Dr. West. He has also become successful in Wall Street and has bought a Mansion in Connecticut burying the memories of the Terror Mask behind him. Meanwhile, in Mexico City, the temple where the mask was found is beginning to pulsate a dark energy, which is seeking out Rick. Rick then receives premonitions from the Terror Mask itself. One night, while Rick and his family are starting to settle into their new mansion, the evil demons that plagued Rick in his youth eventually start to appear again and seem to be going after Rick's family. Suddenly, the Terror Mask beckons Rick once again, telling Rick that the Evil One has once again risen and is now out for revenge for all the times Rick has stopped his evil plans. Knowing fully well that his family is in terrible danger, Rick dons the Terror Mask one last time.

In this single-player action game you control Rick, who has once again donned the accursed Terror Mask to gain the strength needed to enter a haunted house and save his wife (Jennifer) and son (David) from various evil monsters. As Rick you have to race against time through various floors in the mansion and jump, kick, and punch an assortment of monsters to save your wife and child.
Occasionally, Rick is able to pick up weapons (i.e. a baseball bat or an axe, amongst others) to use against the monsters. Once you defeat all the monsters in the room, an in-game map appears to give you some choice in how you get to the end of each level. The game's storyline alters significantly if you fail to finish levels before the allotted time runs out. For example, in the second level, you are tasked with saving Rick's wife Jennifer from being devoured by a boreworm. In order to save her, you must reach the end of the level to confront and defeat the boreworm within the time limit. Failing to do so before time runs out will result in Jennifer being devoured internally by a boreworm planted inside of her body.
This installment of the Splatterhouse series also introduced another power of the Terror Mask. During gameplay, you will periodically find Eldritch Orbs which will fill a power meter at the bottom of the screen when collected. When the appropriate button is pressed while the power meter is charged, you will witness Rick transforming into a hulking and more monstrous version of himself with the Terror Mask being seemingly fused to his head. In this state Rick is significantly stronger and can execute a special attack that involves pieces of his flesh extending from his chest area and acting as a weapon, injuring any monster in the vicinity of their reach. Also, instead of actually wielding the weapons he finds, the mutated version of Rick simply flings them at enemies, dealing a fair bit more damage then swinging the weapons as normal Rick would have.
As was the case in the first two games, the game featured many homages to popular horror films and helped to pave the way for the survival horror genre of video games later seen in Resident Evil series.

NOTE: Unlike the previous games where you can re-use weapons after being hit by an enemy. In this game, The Poltergeist can steal your weapon if an enemy monster hits you.
- Meat Cleaver - A returning weapon from the first game, which does the same thing, chop up monsters.
- 2 X 4 - Another returning trademark weapon from the first game. Although you can't hit monsters into the walls anymore, it is still a great weapon. However, it's a bit slower. While as Monster Rick, it can be thrown like a projectile.
- Axe - The last time this weapon was used was to chop down Evil Cross. Once again this nasty weapon returns in Rick's arsenal.
- Baseball Bat - A somewhat quicker weapon than the 2 X 4, used the same way.
- Cinder Block- A large concrete block that can be thrown at monsters numerous times.
- Butcher Knife - A projectile weapon that can be thrown at monsters.

- Stage 1: 1st Floor Monster - A big mouth monster that assisted the Giant Boreworm in capturing Jennifer. When its encounter, it's seen feasting on flesh of other monsters, but when it spots Rick, the fight really begins. The monster jumps around clawing at its victims. When its head explodes, a giant hand will shoot out from its head to claw at anything in its way. Later, on you meet other monsters like this, but this time they just become fodder.
- Stage 2: Giant Boreworm - The creature responsible for creating the Boreworm Nest from the first game. You finally meet the head Boreworm which was responsible for implanting one of its creatures into Jennifer. When its head is knocked off, mini worms protrude out and becomes its new head.
- Stage 3: Possessed Teddy Bear - Most likely, it's the poltergeist from the previous games, who is disguised as Rick's son, David, reveals its true form then possessed David's teddy bear. After getting the stuffing beaten out of it, it reveals its true form, a demonic walker like creature which can sprints around the room really fast. This boss is probably based on the hallucination scene in the hospital from the anime Akira.
- Stage 4: Embryo/Fetus Monster - This monster guards David, who is attempting to be sacrificed to The Evil One. It first starts out like a bubble, like the baby ghouls from the first game. When it hatches, it grows into a worm like creature which attacks with electricity.
- Stage 5: The Evil One - Rick encountered him in the previous game after he rescued Jennifer from the crystal. He has now crossed into the mortal world and seeks vengeance against Rick and his family. Just like last time, he can shoot lightning bolts from his eyes.
- Stage 6: The Terror Mask - The Mask which helped Rick from the beginning reveals his true intentions to his wearer. He used Rick to destroy The Evil One and his minions from the beginning because The Evil One was the only thing standing in his way to achieve world domination. The Mask's true intentions was to kill Rick's family and to eventually make Rick into a mindless killing machine under its control. The Mask uses its power to create a new body made from the shattered remains of The Evil One to attack Rick, then reveals its true form, which is a giant head wearing the Terror Mask. However, Rick manages to beat him. As the Terror Mask screamed dying, it warns Rick and his family, that as long as human suffering exist, it will continue to live on. It says this only if the player has failed to save Jennifer and/or David. Finally, the Mask cracks and explodes into fragments ending Rick's nightmare...but for how long?
The ending varies depending on whether Jennifer or David live or not.
