New York State Route 104 Truck is a truck route of New York State Route 104 through Rochester. The route was designated to outline a path for trucks to access NY 104 from St. Paul Street, where there is no connection to NY 104 from St. Paul north or from St. Paul south (from the East Ridge Road intersection; the southbound on-ramp is north of Ridge). To alleviate this issue, it is necessary to use East Ridge Road east to North Clinton Avenue to meet NY 104. This path is also the route of NY 104 Truck.

NY 104 Truck follows the former alignment of U.S. Route 104 east along East Ridge Road from St. Paul Street for 0.3 miles, where it turns south onto North Clinton Avenue. After another 0.2 miles, North Clinton meets NY 104 at the first complete interchange on NY 104 heading east from Rochester. NY 104 Truck comes to an end at the eastbound on-ramps for 104 on Clinton.

The segment of NY 104 Truck just east of St. Paul Street to North Clinton Avenue was formerly U.S. Route 104. This designation was removed in the late 1960s.
