As of 2004, the highway's northern terminus is Columbia, Tennessee at U.S. Route 31. Its southern terminus is in Mobile, Alabama at U.S. Route 45.

The highway passes through the following states:

• Tennessee
• Alabama. Between Mobile and Fayette and again from Killen to the Tennessee state line, U.S. 43 is paired with unsigned State Route 13. Between Fayette and Hamilton, U.S. 43 is co-routed with unsigned State Route 171. Between Hamilton and Florence, the unsigned partner for U.S. 43 is State Route 17.
