Undertow is Tool's first full-length album, released in 1993. Controversy was generated by the photos in the liner notes of obese nudes underwater and the band members with pins in the sides of their heads, resulting in the album being removed from most large retail chains. The spine of the CD case features a smaller picture of the obese woman from the booklet. Some versions of the album replaced the cover image with a large barcode and included a note from the band (see below right).
Undertow was certified double platinum by the RIAA on May 14, 2001.

- When the cover is held up to a light, the obese woman inside is seen encircled by it.
- Behind the CD tray is a picture of a cow trying to lick itself, probably a reference to Bill Hicks' comment, "Heaven is in a cow's ass."
- Henry Rollins' spoken word piece on the album version of "Bottom" is different than the one Maynard James Keenan recites live.
- "Disgustipated" is a hidden track that appears as track 69 on early U.S. pressings, track 30 on others, with the intervening tracks as three seconds of silence each. On European and other releases, it appears as track 10, directly following "Flood". On some pressings, it is tacked onto track 9, after "Flood" and a period of silence.
