The 2003-04 season of the European UEFA Champions League football club tournament was won by FC Porto in a comfortable final victory against AS Monaco, following up their 2003 UEFA Cup success.
This was the first UEFA Champions League competition which eliminated the second group stage in favour of a knockout format for the round of 16.

16 winners from the third qualifying round, 10 champions from countries ranked 1-10, and 6 second-placed teams from countries ranked 1-6 were drawn into 8 groups of 4 teams each. The top 2 teams in each group will advance to the Champions League play-offs, while the third-placed teams will advance to round three of the UEFA Cup.
Tiebreakers, if necessary, are applied in the following order:
- Points earned in head-to-head matches between the tied teams.
- Total goals scored in head-to-head matches between the tied teams.
- Away goals scored in head-to-head matches between the tied teams.
- Cumulative goal difference in all group matches.
- Total goals scored in all group matches.
- Higher UEFA coefficient going into the competition.
Teams that have clinched a spot in the Round of 16 are indicated in bold type.
Teams that have clinched a spot in the UEFA Cup are indicated in bold italics.
Teams eliminated from Europe are indicated in plain italics.
